Most institutions do not actually require a letter from the clergy; they only require supporting documentation.
I have written one that you can download and print out, then fill in the blanks and submit it to your employer.
All I ask in return is that you read the letter and make sure you agree with its meaning and intent before you file it with your exemption paperwork.
